2. Visit the Local Dukcapil Office
Visit the Dukcapil office responsible for the deceased’s last registered address. Submit all required documents and request the issuance of the death certificate.
3. Receive the Official Death Certificate
Dukcapil will issue a printed and signed death certificate. The new version is issued with a QR code.
